Questionnaire


 

Questionnaire allows you to create questions with multiple answers in a service desk project. These questions are visible whenever a request is submitted in that project. This is used to get additional information when either records are being submitted or states in transition.

Name

Enter the Name. This will be the label of the Questionnaire in the Submit/Edit page.

Applicable States

Select the states for which the Questionnaire must appear.

Service

 

Select the services for which the Questionnaire must appear.

Category

Sub Category

Derived fields

Select the categories and sub categories for which the Questionnaire must appear.

Check the box if derived fields need to be considered and select the derived fields.

Is Mandatory

Check the box 'Is Mandatory' if it is mandatory for the answer to be provided while submitting/changing state of the request.

Question

Option

Enter the question and enter the option and click 'Add Option'. Similarly, you can add multiple options and click 'Add Option' each time. When you are done adding options, click 'Add Question' to add the question.
